100 Club of Chicago - Application for Membership

When you become a 100 Club member you are able to assist those who protect us every day. 100 Club members have the knowledge that when a law enforcement officer, firefighter, emergency medical service personnel or federal agent is killed in the line of duty in Cook County, the 100 Club is there to assist the family.

We have several membership options to choose from. Click on the membership level of your choice, fill out the application. Upon receipt of your membership application and payment, you will receive a membership packet which will include a welcome letter, membership card, decal, and a static sticker. Periodically you will receive an update with news about the club and those whom we have helped with your support.

• Individual Membership - $200.00
  
  
• Joint Membership (2) - $300.00
  Great for couples who want to support the 100 Club.
  
  
• Family Membership (4) - $500.00
  Great for a family of four that wants to show their support.
  Choose to pay quarterly, semi-annual or annually.
  
  
• Corporate Membership - $1,000 to $25,000
  Corporate Members can choose from the basic level of $1,000 up to a 5 Star Membership of $25,000.
  
  
• Lifetime Membership - $2,500.00
  Proudly show your life long support of the 100 Club.
  
  
• Lifetime Joint Membership - $3,500.00
  Couples can show their life long support of the 100 Club.
